OSLC Youth and Kids Registration Form

January 1st - January 1st, 2028

This form is our main source of information, medical release, photo release and authorizations for your youth or child to participate in any activity or event at Our Savior Lutheran Church. This form shall remain effective through August 31 following your child's high school graduation.

It is the responsibility of the undersigned parent/guardian to inform OSLC of any changes in address, phone, medical insurance and/or medical conditions. This can be done by contacting our Youth and Kids Ministry assistant.

This form does not register your child for VBS
Some activities such as camp and VBS may also require a separate registration form.

Please note that the dates and times on the right column indicate the time span of events covered by this form rather than the dates of actual events.
