OSLC Participation Form

IF YOU ARE FILLING THIS OUT FOR A MINOR, please enter the name and information of your child in the fields. If you are logged in and your name is autofilled on the form, please log out and refresh the form so you can enter your child's information. Unfortunately, you cannot fill out a form for someone else in the Churchcenter App at this time. You will have to use a web browser to complete it.

This form is our main source of information, medical release, photo release and authorizations for children, youth, and adults to participate in activities or events with Our Savior Lutheran Church. For minors, this form shall remain effective through August 31 following your child's high school graduation, or until they turn 18 years of age. For adults, this is just a one time permissions form. It is the responsibility of the undersigned parent/guardian or individual to inform OSLC of any changes in address, phone, medical insurance and/or medical conditions. This can be done by contacting our Discipleship Assistant.

Please note: - Some activities such as camps, retreats, and VBS may also require a separate registration form. Completing this form does not register you for those events. A list of upcoming events can be found here.
