OSLC Volunteer Application

Volunteer leaders play incredibly important and significant roles as representatives of Jesus and OSLC. All leaders are required to submit an application and be appropriately screened and trained before serving in any volunteer capacity at OSLC.

These references should preferably be individuals from organizations where you've volunteered or performed similar work in the past rather than personal or family references. Minors without a work or volunteer history should list other adults they've worked alongside in an organizational setting (teachers, coaches, ministers, etc.).

A disqualifying offense that will keep an individual from working with minors will be determined by the responsible ministry director on a case-by-case basis in light of all the surrounding circumstances. Generally, convictions for an offense involving minors and/or for offenses involving violence, dishonesty, illegal substances, indecency and any conduct contrary to our mission will preclude someone from being permitted to work with minors. Failure to disclose a criminal conviction on the application form will also be a disqualifying event.

The facts set forth in my volunteer application are true and complete. I understand that if I become an OSLC volunteer, false statements on my application shall be considered sufficient cause for dismissal.

I authorize the use of any information in this application and any attached supplements to verify my statements, and I authorize the past employers, schools, churches, all references, and any other persons or organizations, whether or not identified in this application, to answer all questions asked concerning my ability, character, reputation, and previous employment record. I release all such persons from any and all liability or damages on account of having furnished such information.

A national criminal background check is required for all volunteers (18 and older) who work with/or alongside minors and will be administered electronically through Checkr. If the position I am applying for meets those parameters, I will be contacted by the responsible ministry leader and will receive a link to the background check through a separate email from Checkr. I will complete this upon receipt as the link is time-sensitive. I understand that background checks are valid for 2 years.

Upon completion of the application, a face-to-face interview may be scheduled with me to discuss my suitability for the role with the responsible ministry director.
